VERSION 9.9.3 of Visual RMS RELEASED

(MUST be at version 9.9.1 or later for this update to be successful.)

 

On September 30, 2010 version 9.9.3 was released by P&NP. It only updates those files released since 9.9.1, therefore you must be at that version or later PRIOR to attempting to put on 9.9.3. The biggest change from Version 9.9.2 to Version 9.9.3 is the implementation of MDS 3.0 and Resident Observations 3.0. This update does NOT contain the necessary changes to implement CATS in the care plan module. That will more than likely be released sometime next week.

Please follow the directions below to successfully install 9.9.3.

  1. Before each and every update, be absolutely certain that a backup exists which may be restored fully should it be necessary. Backups are extremely critical at all times as more and more critical information is being moved from paper to electronic media and the reliance on that electronic media is becoming imperative.

  2. Save the file UPD993.ZIP and place it in the Visual RMS folder. This can be accomplished by right clicking on the file name and selecting "Save Target As...".

  3. Now uncompress the file and it should create a folder called UPD993.

  4. Before you start installation, know where your VISRMS system is installed. Check the properties of your desktop icon if you are not sure of the location.

  5. Now type the following command: UPD993\SETUP. This will execute the setup command, just follow the prompts and the update will be installed.

    The first prompts that you will see allows you to select the drive and folder where the system is located. You must select the proper drive and folder as you located in step 5.

    Once you accomplish this, it should say that it will overwrite files and ask whether you wish to continue. Only select yes if the backup in step 1 was successful, otherwise answer no and go back to step 1.

  6. Once you are sure that the update was installed successfully, then you may erase the UPD993 subfolder of Visual RMS.

VERSION 9.9.3f of Visual RMS RELEASED

(MUST be at version 9.9.3, 9.9.3a, 9.9.3b, 9.9.3c, 9.9.3d or 9.9.3e for this update to be successful.)

This update does replace the previously released 9.9.3a, 9.9.3b, 9.9.3c, 9.9.3d and 9.9.3e.

 

On December 8, 2010 version 9.9.3f was released by P&NP. It only updates those files released since 9.9.3, therefore you must be at that version PRIOR to attempting to put on 9.9.3f. This update includes everything which was in the release 9.9.3a AND 9.9.3b AND 9.9.3c AND 9.9.3d AND 9.9.3e PLUS a few other corrections/ enhancements. The changes which are included in this update are:
  • Data for the UB-04 from MDS was not properly populating in the claim details. (9.9.3a)
  • Corrected typo on the MDS selection screen. It said 15-day rather than 14-day. (9.9.3a)
  • Implemented the link to look back into Infection Tracking module for immunization information as it was in the MDS 2.0 (9.9.3a)
  •  Implemented the suggestion on payor source in Section S as it was in the MDS 2.0 (9.9.3a)
  • Put in a “+” sign for the Medicaid number if the person is currently pended (9.9.3a)
  • Try to fill in prior assessment information in section V if possible (9.9.3a)
  • Corrected the print out which was not printing field G0110d1 (9.9.3a)
  • Data in column C of Question V0200 was not being saved properly (9.9.3a)
  • Fill in text fields at the time of submission with "^" which are left blank (9.9.3a)
  • Added the tracking forms to the in progress report (9.9.3a)
  • Loop back to the assessment selection screen after editing an assessment (9.9.3a)
  • Incorrectly interpreted the data in order to calculate the dental trigger (9.9.3a)
  •  Incorrect verification of A0600b when checking a Railroad insurance number
  •  (9.9.3a)
  • CATS implementation in the MDS Care Plan module (previously named RAPS Care Plan) (9.9.3b)
  • Enhancements have been made to the MDS form for filling in the information including skip pattern enhancements, error checking, etc. (9.9.3c)

  • Care plans have been updated with everything which has been brought to our attention to date. One thing to note that the bolding of triggers is fully operational at this point (9.9.3e)
  • Can now run the MDS in Progress report for just those items ready to be submitted (signed off with 0 errors) (9.9.3e)
  • New report under Submission Assistance (Report on Assessments Ready) which is just another way to run the report outlined above (9.9.3e)
  • New item under Submission Assistance (Batch Submit) has been added. There will be an additional fee for licensing of some 3rd party software required to enable this option on the menu (9.9.3e)
  • Implementation of the new CMS-672/CMS-802 have been added under the menu item CMS Forms in the MDS module (9.9.3e)
  • Further enhancements have been made for the MDS Assessment Check. Please note that this will NOT go back and review any assessments already existing. It will only do the additional checks when saving an assessment or when you ask to view the error report. (9.9.3e)
  • Further enhanced the record selections for the PPS Scheduling Aid, PPS adjuster and UB-04 (9.9.3e)
  • Expanded the Discharge Status field to 2 characters for the MDS 3.0 data. This report was in error in that it only showed the first character of A2100. (9.9.3f)
  • Typo in the check of J0200-J0850 when B0100=1. Looked at variable J rather than I. This will be properly processed with this update. (9.9.3f)
  • Corrected situation where after doing D0300…when it carated out D0500 it did not carat out D0650 also. This will be properly processed with this update. (9.9.3f)
  • Prior to this update, people out on bed hold were left off of the CMS-802 form (9.9.3f)
  • After copying assessment as a new one...forgot to blank out the key date fields V0200b2, V0200c2 and Z0500b. People were getting a message saying that the assessment was ready for submission when they had just created it. (9.9.3f)
  • Blank out fields which are not part of the assessment and may have been copied from a previous one. Some issues were found when the assessment which was copied had answers to questions which are not asked on the current form. (9.9.3f)
  • Corrected situation of writing out values which were not changed by this user even though the answer they have is different than the one which currently resides in the tables. The system will only save answers which have been explicitly changed by this user. (9.9.3f)
  • Modified to handle "-" or "^" in M0610 while doing the print out. Prior to this M0610 would be 00.0 on the print out when the values were a dash or carat. (9.9.3f)
  • Validation report was changed by CMS in that they are now capitalizing the value submitted in Fac_Doc_Id. Have to force to lower case for our check. (9.9.3f)
  • Added the ability to use the Validation Utility Tool supplied by CMS. You can now right click on the assessment in the selection screen and choose the option to run the Validation Utility Tool if you so choose. (9.9.3f)
  • PPS Scheduler for MDS 3.0 was not handling insurance changes to/from Medicare consistently (9.9.3f)
  • Section O when you use our Resident Scheduling Module can now pull the data from that module after you review/accept it.
  • Modified the frequency for which libraries are checked for updates and checking for valid date/time on the computer. This will decrease the time necessary to start up our application on slow machines. (9.9.3f)

VERSION 9.9.4 of Visual RMS RELEASED

(MUST be at version 9.9.3 or later for this update to be successful.)

 

On February 24, 2011 version 9.9.4 was released by P&NP. It only updates those files released since 9.9.3, therefore you must be at that version or later PRIOR to attempting to put on 9.9.4. The changes which are included in this update are:

Menu system – Now have the ability to go directly from one module to another without having to quit back to ADT first

Change to UB-04 for Medicare – Moved the assessment date information from F45 to an occurrence code 50

PPS Schedules – reworked the way that significant changes/corrections are handled and their impact on the schedule

CAAs – Implementation of the Care Area Assessment add-on for the MDS v 3.0 module. This has been added for facilities who have ordered this module already. Documentation has been placed in the manual folder (CAA Documentation.PDF)

CheckLibs Application – new application which will verify that the current work station has all of the necessary libraries properly installed. You should alert your IT to this valuable addition

Inactivation Tracking Form – created a new form so that people can print out inactivations for tracking purposes. This form does NOT exist in the federal lists of approved forms released by CMS.

PNPSupport Application on Menu – Added the PNPSupport Application to the ADT menu under utilities to assist people in starting this up for support.

IMPLEMENTATION NOTES: With the release of 9.9.4 there are many updated libraries. To the user this means that someone needs to get into Visual RMS on each workstation with administrator privileges (Remember on Vista or Windows 7 machines you must select to “Run As Administrator” if the UAC is enabled). Alternatively you can use the new CheckLibs/CheckLibs9 application found in the main folder containing our software to assist in this process.

Vista and Windows 7 workstations should really be using the “version 9” of our programs (i.e. VisRMS9.exe, CheckLibs9.exe, Unsubmit9.exe, etc.). This is really optional on workstations running a version other than Vista or Windows 7. In order to implement this you will need to run the “version 9” of the client (found in the CLIENT subfolder called SETUP9.exe). Once you have successfully installed the “client”, then it is just a matter of updating the shortcuts on the workstation to run the “version 9” program desired. The use of the “version 9” of our software is VERY IMPORTANT on Vista/Windows 7 machines.

Please follow the directions below to successfully install 9.9.4.

INITIAL Release 2011 of Visual RMS

(MUST be at version 9.9.3 or later for this update to be successful.)

 

On March 28, 2011 version 2011.0 was released by P&NP. It only updates those files released since 9.9.3, therefore you must be at that version or later PRIOR to attempting to put on 2011.0. The changes which are included in this update are:

 

Added gender to the New Admit report in ADT

 

MDS 3.0 has undergone some major changes required by CMS and NYS. Some of these changes will be visible and others will not. Some of these changes are:

1)      New section S on all assessments dated 4/1 or later

2)      Handle the ability to input an assessment for Medicare C (or Medicare Advantage) plans and NOT have them submit to CMS and NY

3)      The assessment selection screen for any assessments made from the time the update is put on forward will now show “errors” in three categories. Previously it would just tell you that you had 4 errors for example. Now it will show as 2/1/1 meaning there were 2 fatal errors, 1 warning and 1 informational message. Assessments which do not have “fatal” errors and are complete may now be submitted even though there are warnings/informational messages.

4) Form changes on section S required by the state and the additional of some questions to other sections mandated by CMS

NOTE: Visual RMS will suggest that S8055 be filled in as Medicare for all Medicare C residents (where the insurance is properly set up) as per the initial instructions from NY. We have asked the state to reconsider these instructions and bring the definition of Medicare in line with the Federal definition so that the form is not conflicting with itself.

Rewrite of the PPS Scheduling Aide (and is also reflected in creating an UB-04 and/or PPS Adjustment in the billing module. The main difference is that it better handles when there are multiple assessments in a given window AND it will handle when you do an assessment too early or too late.

 

A new report which will give a “true” average case mix has been added to the MDS 3.0 module. Please note…depending on the size of the facility and the length of time that you ask for the averages…it can take considerable time to calculate. The report will calculate who was in the facility for each day and then use the NY Medicaid style rules of selecting an assessment for that time period for a CMI. The CMI can ONLY be done using the NY Medicaid weights since there is no federal case mix.

 

The total line on the UB-04 will only fill in on the last page of a multi-page claim. For all of the other pages the total line is left blank.

 

Modified the clinician note module to standardize the way that progress, treatment and physician notes are completed. For clinicians which use these notes with a level of 5…they will now be able to right click on the header of the form which shows the list of notes and select a filter on discipline and/or clinician who wrote the note. This should make it easier to find the note which you are looking for.

 

 

IMPLEMENTATION NOTES: Particularly for facilities which use our clinician notes module. This update can take some time to put on. Do NOT abort the process once it has started.

For EVERYONE…make sure a valid backup has been done PRIOR to the installation of this update in case power is lost, etc.

Any assessments which already exist for after April 1, 2011 PRIOR to putting on this update are in jeapordy of NOT using the correct form for submission. As we warned you last week, these assessments should be deleted and new ones created once the update is installed.

UPDATE Release 2011.1 of Visual RMS

(MUST be at version 2011.0 for this update to be successful.)

 

On April 1, 2011 version 2011.1 was released by P&NP. It only updates those files released since 2011.0, therefore you must be at that version or later PRIOR to attempting to put on 2011.1. The changes which are included in this update are:

Accident/Incident – Expanded some of the reports to include upto 7 categories. Note: If you select more than 5, then the report may come out landscape in order to have room for the additional 2 categories.

 

Billing – UB04 creation. An error was occurring when trying to create a claim for September, 2010 when it was gathering information from possible MDS 3.0 records for the transition.

 

MDS 3.0 – Validation – Problem was found when comparing dates to the DOB because of the change in century

 

MDS 3.0 – Validation – Problem was found in the CMS specifications for not allowing special characters in X0400

 

MDS 3.0 – PPS Schedule – Problem was found where sometimes the records would not be reported chronologically for a resident

 

MDS 3.0 – MDS in Progress – Problem was found in generating the report when looking at the expanded number of error column

 

MDS 3.0 – Setup of Questions Selection for Clinicians – Problem was reported with a typo in it saving the data out to the two versions which exist now (pre- and post- 4/1)

 

MDS 3.0 – Assessment Selection Screen and selecting EXPORT – a problem was found in this screen due to it not selecting the appropriate file

 

MDS 3.0 – Right clicking on a date field and selecting the entry -------- - An error was being generated in not being to locate the three appropriate fields to put the dashes (Year, Month and day)

 

MDS 3.0 – Modification – Problem with the new check to make sure that fields in X match those in A as they can no longer be changed with a modification.

 

Resident Scheduling – Event List – a problem was reported in generating the report

 

Reports in general – some people were reporting issues with the program having an error when it was getting the printer information.

Installation Notes: This update is small and in most cases will take less than 1 minute to install. Once you have installed 2011.0...do NOT attempt to install it again accidentally thinking it was this update 2011.1!

This update has been incorporated into 2011.5. Just follow instructions for that update.

UPDATE Release 2011.2 of Visual RMS

(MUST be at version 2011.0 for this update to be successful. You do NOT have to have 2011.1 installed because 2011.2 includes everything which was in 2011.1)

 

On April 14, 2011 version 2011.2 was released by P&NP. It only updates those files released since 2011.0, therefore you must be at that version or later PRIOR to attempting to put on 2011.2. The changes which are included in this update are:

 

1)      Improved the accuracy of the fingerprint system for electronic signatures

2)      Improved the speed of drawing the MDS 3.0 assessment screen

3)      Corrected an issue when some people wanted to print an MDS and the system would no longer respond. This only pertains to those facilities which use our electronic signature.

4)      Clinician Notes – forgot to limit the report to just the category of note wanted (progress, treatment, physician)

5)      When working with just section V…only validate the signature to section V…do not recalculate the other signatures. This only pertains to those facilities which use our electronic signature.

6)      Statutory scheduling was having issues with imported MDS data

7)      Tried to validate signatures when the person never actually e-signed. This was only problematic for facilities where some clinicians electronically signed off and others did not.

8)      Updated the MDS 3.0 Validation Utility Tool distributed by CMS. The updated version will correctly handle the new form. Note: To use the Validation Utility Tool, the computer must have a version of JAVA actually installed.

9)      Fixed a problem with report generation when the printer is off.

Installation Notes: This update is small and in most cases will take less than 1 minute to install. Once you have installed 2011.0...do NOT attempt to install it again accidentally thinking it was this update 2011.1 or 2011.2!

This update has been incorporated into 2011.5. Just follow instructions for that update.

UPDATE Release 2011.3 of Visual RMS

(MUST be at version 2011.0 for this update to be successful. You do NOT have to have 2011.1 or 2011.2 installed because 2011.3 includes everything which was in those updates)

 

On April 19, 2011 version 2011.3 was released by P&NP. It only updates those files released since 2011.0, therefore you must be at that version or later PRIOR to attempting to put on 2011.3. The changes which are included in this update are:

Address the resolved issues as per the NYS DOH Dear Administrator Letter dated 4/15/2011 as it pertains to the MDS 3.0 Assessment system.

This update has been incorporated into 2011.5. Just follow instructions for that update.

UPDATE Release 2011.4 of Visual RMS

(MUST be at version 2011.0 for this update to be successful. You do NOT have to have 2011.1, 2011.2 or 2011.3 installed because 2011.4 includes everything which was in those updates)

 

On May 6, 2011 version 2011.4 was released by P&NP. It only updates those files released since 2011.0, therefore you must be at that version or later PRIOR to attempting to put on 2011.4. The changes which are included in this update are:

  • CAA system would cause an error when creating the “Triggered Report” showing what areas had “triggered” on the assessment
  • Accident/Incident had an issue when selecting 7 categories which it was just expanded to with 2011.1.
  • Corrected issue when checking dates in the MDS as it was trying to validate a date field which was not on the currently selected form.
  • Problem with MDS v 3 module not properly blanking out inactive fields (fields which are not on the currently selected form)
  • Calculating a RUGs-III score for an MDS 3.0 for Medicaid had an error when calculating the Cognitive Performance Score when the cognitive questions are not answered. They are either skipped (“^”) or not assessed (“-“)
  • An error was generated when doing the PPS Scheduling Aid in MDS v 3 for a period which preceeded the use of MDS v 3 (prior to 10/1/2010)
  • New copyright notice as required by CMS has been added to the last page of the “old” assessments. There is still no copyright notice on the new forms which were started to be used effective 4/1/2011.
  • Problem was found where if the only thing marked in ADT for race/ethnicity was “Hispanic or Latino” that the face sheet would put it out as other. This is due to this was not a possible combination previous to the implementation of MDS v 3. If this occurs now, it will mark them as White/Hispanic for the purposes of the face sheet. 

This update has been incorporated into 2011.5. Just follow instructions for that update.

UPDATE Release 2011.5 of Visual RMS

(MUST be at version 2011.0 for this update to be successful. You do NOT have to have 2011.1, 2011.2, 2011.3 or 2011.4 installed because 2011.5 includes everything which was in those updates)

 

On June 9, 2011 version 2011.5 was released by P&NP. It only updates those files released since 2011.0, therefore you must be at that version or later PRIOR to attempting to put on 2011.5. The changes which are included in this update are:

  • Hospice report in ADT will now print out in Alphabetical order

  • Generic Face Sheet – for those people who have a printer which cannot handle the Generic Face Sheet. The system will now recognize that fact and output it to a PDF. As a PDF people can select the option to “shrink to fit” their printer capabilities.

  • Corrected an issue when going back and pulling assessment information from MDS v 2. This pertains to the UB-04 as well as the PPS Scheduling Aid.

  • EOT was expecting to be day following end of therapy rather than allowing it to be 1-3 days following as required. This pertains to the UB-04 as well as the PPS Scheduling Aid.

  • Modified handling of the VOID period when it is effecting observation dates for an assessment. This pertains to the UB-04 as well as the PPS Scheduling Aid.

  • Added a check to make sure that an “other” diagnosis is not the same as the principle diagnosis on the UB-04

  • Another problem was found when unexpected data was coming through on the Audit report of the Accident/Incident Module. This condition is now being trapped and reported properly

  • Added the Urban/Rural flag to the MDS v 3 Setup table rather than pulling it from the MDS v 2 Setup table. This also means that it has been added to the facility setup form

  • Corrected an issue where occasionally users were receiving an error while trying to print the MDS v 3 form after editing

  • A loose interpretation of converting from MDS v 2 Quality of Care/Quality of Life indicators to using the MDS v 3 was done. This is a totally unofficial project since CMS has not released any information in regard to this. At this time, because it is “our” interpretation of what is going to happen this product is temporarily being supplied free of charge. The only support for this will be via e-mail while the implementation of this progresses, people find it useful or not, and whether CMS ever releases the "true" indicators.

  3. Now uncompress the file and it should create a folder called UPD2011-5. Make sure you go into the correct folder as you must NOT install the inital release of 2011.0 or any other ealier release of 2011 again!
  4. Before you start installation, know where your VISRMS system is installed. Check the properties of your desktop icon if you are not sure of the location.
  5. Now type the following command: UPD2011-5\SETUP. This will execute the setup command, just follow the prompts and the update will be installed.

    The first prompts that you will see allows you to select the drive and folder where the system is located. You must select the proper drive and folder as you located in step 4.

    Once you accomplish this, it should say that it will overwrite files and ask whether you wish to continue. Only select yes if the backup in step 1 was successful, otherwise answer no and go back to step 1.

  6. Once you are sure that the update was installed successfully, then you may erase the UPD2011-5 subfolder of Visual RMS.

Generic Updates for RMS

     The ICD-9-CM codes which are to be used effective October 1, 2010 have been released. You must start using these codes by that date. As was true for 2004-2010, there is no grace period for their implementation and use.  Please download the file by right clicking the file name and choosing save target as and place it in the main Visual RMS folder. Once this is complete, uncompress the file and make sure the three files enclosed (ICD9LIST.DBF, ICD9LIST.CDX and ICD9LIST.FPT) are put into the database subfolder of the Visual RMS system. In order to be successful in doing this...nobody can be accessing the ICD-9-CM list at this time.

2011 HCPCS Codes

(MUST be at version 9.9.3 or later for this update to be successful)

On December 31, 2010 the HCPCS codes to be used for 2011 were released by P&NP. This is just an interim release and only updates the HCPCS/CPT-4 codes which are to be used effective January, 2011.

  1. Before each and every update, be absolutely certain that a backup exists which may be restored fully should it be necessary. Backups are extremely critical at all times as more and more critical information is being moved from paper to electronic media and the reliance on that electronic media is becoming imperative.

  2. Save the file UPDHCPCS.ZIP and place it in the Visual RMS folder. This can be accomplished by right clicking on the file name and selecting "Save Target As...".

  3. Now uncompress the file and it should create a folder called UPDHCPCS.

  4. Now type the following command: UPDHCPCS\SETUP. This will execute the setup command, just follow the prompts and the update will be installed in most cases in less than one minute.

  5. Once you are sure that the update was installed successfully, then you may erase the UPDHCPCS subfolder of Visual RMS.

Microsoft Visual FoxPro version 8 Client

 

P&NP will soon be releasing version 9.5.0. Starting with this release is the expectation to start using Visual FoxPro version 8 rather than the version 6 which is currently in use. In preparation of this, large facilities may find it useful to start implementing the client now rather than later. It is a simple Setup routine which will copy the needed files and set them up so that the machine will be ready for 9.5.0 when it is released. It will have absolutely no effect to any other software currently installed on the machine.

EMS

VERSION 4.3.0 of Visual EMS RELEASED

On November 17 version 4.3.0 was released by P&NP. This updated version fixes several small bugs and includes many updated features. For a list of changes read the EMS430.TXT file after you update.

1) Save the file UPD430.ZIP and place it in the Visual EMS folder. This can be accomplished by right clicking on the file name and selecting "Save Target As...".

2) Now open a DOS Prompt (Command Prompt) and change the folder to the Visual EMS folder. As an example, people with a network and Visual EMS is installed in F:\VEMS would type: F: and hit the enter key, then they would type CD \VEMS and hit the enter key.

3) Now type the following command: PKZIPC  UPD430  -EXTRACT  -DIR. This will expand the enclosed files into a subfolder of Visual EMS called UPD430.

4) Now type the following command: UPD430\SETUP. This will execute the setup command, just follow the prompts and the update will be installed in most cases in less than one minute. Now when you go into Visual EMS it should say that you are at version 4.3.0. If you are not, then try these steps again or give P&NP a call for assistance.

5) Once you are sure that the update was installed successfully, then you may erase the UPD430 subfolder of Visual EMS.

On September 2 version 1.5.2 was released by P&NP. Visual Fixed Assets must be at version 1.5.1 in order for this to install properly.

  4. Now type the following command: PKZIPC  UPD152  -EXTRACT  -DIR. This will expand the enclosed files into a subfolder of Visual Fixed Assets called UPD152.

  5. Now type the following command: UPD152\SETUP. This will execute the setup command, just follow the prompts and the update will be installed in most cases in less than one minute. Now when you go into Visual Fixed Assets it should say that you are at version 1.5.2. If you are not, then try these steps again or give P&NP a call for assistance.
